Immunization requirements

Do I have to provide proof of immunization?

Minnesota Statute 135A.14 requires proof that all students born after 1956 are vaccinated against diphtheria, tetanus, measles, mumps and rubella (MMR). Any non-exempt student who does not provide the required immunization information within 45 days after the first enrollment cannot register. Complete an electronic immunization record.
